Angiotensin-converting enzyme 2 (ACE2)

Angiotensin-converting enzyme 2 (ACE2) is a membrane receptor expressed on the surface of airway epithelial cells. ACE2 was intensively studied in the early 2000's because it binds with the Spike protein of coronaviruses SARS-COV and HCoV-NL63. This interaction is mediated by the RBD domain of the spike protein, and is believed to be the pivotal event in the membrane fusion process. Recently, ACE2 has come back to the spotlight amid the outbreak of another deadly coronavirus, SARS-COV-2 (formerly 2019-nCoV). It's been shown that SARS-COV-2 also uses ACE2 as the receptor for host cell entry. This finding suggests that blocking the interaction between the S protein and ACE2 is a valid approach to prevent or treat coronavirus infection.

In theory, both the SARS-COV-2 virion and the surface ACE2 can be the target for therapeutic interventions. A soluble spike protein or a blocking antibody can be used to block ACE2, and by so doing, prevent the viral infection. On the other hand, a soluble ACE2-Fc fusion protein may be used to block the S1 protein on the virion, achieving the same result.

It's noteworthy that in addition to mediating host cell entry, ACE2 may play other roles in the pathogenesis of respiratory diseases. ACE2 is an important regulator of the renin-angiotensin system (RAS). ACE2 negatively regulates the activity of angiotensin II by converting it to angiotensin 1-7. Dysregulation of ACE2 has been observed in lung injuries caused by RSV and avian flu (H5N1) infections.The administration of ACE2-Fc fusion protein has been shown to alleviate the respiratory symptoms in animal models.
